Ordering a test is simple.
Please call 1 (888) DNA-CODE or 1 (304) 690-4361 for customer service assistance.
Customer Service will ask you for the names, addresses, and phone numbers of the parties to be tested. Our friendly staff will answer your questions and provide information about the process.
We will assist you with scheduling appointments for sample collection at the nearest collection site. You will need to take the following to the sample collection site:
- Picture identification of the mother and alleged father
- Birth certificate of the child
- Proof of custody or guardianship for child, if appropriate
- Payment for services at the collection site
You will need to sign a Consent Form and adults must provide a thumbprint. The collection site will overnight the samples sealed in packages provided by our laboratory to ensure chain of custody is protected.
Services & Fees
We require an initial, non-refundable, deposit of $100 before a collection site is identified and DNA testing begins. Clients are responsible for payment for sample collection at the collection site, which generally ranges from $20-$45 per person (the collection fee is seperate from our testing fees). Before results are released, we must receive full payment for the balance of services.
We accept money orders, cashier's checks, VISA and MasterCard.
